For Eating Disorder or Disordered Eating Support:

However, if you have concerns that haven’t been addressed in this self-assessment, please follow up with your primary care provider and let them know your concerns so they can guide you.
For Substance Use Treatment:
We recommend that you and your doctor put in a referral to a substance use treatment program as soon as possible.
